Opioid Conversion Chart
Opioid Conversion Chart Lanyard Card
This card features an opioid conversion chart, to be able to convert daily doses of common opioid analgesics to the morphine daily equivalent; the so-called oMEDD. This is a must have for anyone that treats pain, and for clinicians performing an opioid rotation.
It also has a table of approximate equianalgesic doses of common opioids.
Please note: It is the responsibility of the prescriber to understand opioid rotation and calculate the doses, taking into account the patient’s particular PK/PD and concurrent medications. This card is an aide memoire only and may be inaccurate in some situations. It is the responsibility of the prescriber to ensure the contents of this card is correct and up to date before using.
